Orlistat is a specific inhibitor of gastrointestinal lipases, which carry out the digestion of fats. The drug has a high lipophilicity and is mixed with drops of fat. Orlistat covalently binds to the active center of pancreatic and gastric lipases, thereby inactivating them. In view of the inhibition of gastrointestinal lipases, triglycerides can’t enter the blood. This creates an energy deficit, which leads to the mobilization of fat from the depot. Under the influence of the Xenical, the mass of visceral-abdominal fat decreases. Orlistat also helps reduce hypercholesterolemia - in view of the decrease in the amount of free fatty acids and monoglycerides in the gut lumen - the solubility of cholesterol decreases, which reduces its ability to penetrate the blood. Due to the normalization of the lipid composition of the blood, the sensitivity of the tissues to insulin increases, the hyperinsulinemia decreases, which also contributes to a decrease in the mass of visceral-abdominal fat. It is also applied in the treatment of type 2 diabetes and metabolic syndrome, because with this substance, compensation of carbohydrate metabolism can be achieved.
